Social Care Network Screener/Navigator

At CMCS (Cardinal McCloskey Community Services), our mission is to protect, empower and promote independence for at-risk Children & Families and those with developmental disabilities through quality community-based services.

The Social Care Network Screener/Navigator works independently and as a part of a larger team to provide screenings, navigation to services and case management for eligible Medicaid participants as they relate to their social determinants of health needs like housing, food, and transportation. This role involves conducting screenings, eligibility assessments, and facilitating referrals to appropriate programs, both within the Social Care Network and within the broader community. Creates and updates care plans to address individuals' specific needs and goals. Provides training and support to clients and staff regarding the Social Care Network and its services.

Requirements:

• Bachelor's Degree
• Minimum of 2 years of experience in social services, case management, or care coordination.
• Strong understanding of internal referral systems and multidisciplinary team collaboration.
• Excellent communication, organizational, and documentation skills.
• Proficiency in using Electronic Health Records and Microsoft Office Suite.
• Spanish speaking required.
