Position Summary

Under the direction and supervision of the Director of Nursing Services, the Medicare/MDS Coordinator is responsible for notifying and coordinating the Interdisciplinary Team (IDT) for MDS assessment completion by state and federal regulations. This role plays a vital part in ensuring accurate resident assessments and quality care planning within a long-term care setting.

General Duties and Responsibilities

Clinical

  1. Coordinates the Medicare/MDS resident assessment process.
  2. Ensures timely completion of MDS assessments by the Interdisciplinary Team.
  3. Coordinates the development, implementation, and evaluation of individualized plans of care.
  4. Administers or implements treatments, medications, or nursing interventions as indicated by the plan of care or physician orders.
  5. Provides nursing care by infection control standards.
  6. Adheres to safety policies when delivering nursing care.
  7. Initiates emergency measures as needed by center policy and nursing standards.

Administrative

  1. Facilitates the exchange and utilization of essential information to support quality resident care.
  2. Maintains require documentation in compliance with federal, state, and company policies.
  3. Coordinates and participates in assigned meetings and in-service training.

Consumer Service

  1. Projects a professional image through appropriate attire, behavior, and speech.
  2. Adheres to company standards for addressing and resolving consumer concerns.
  3. Protects the rights and dignity of all residents.

Qualifications

  1. Current and valid RN license required.
  2. Ability to communicate effectively in English, both verbally and in writing, as a business necessity.
  3. Strong administrative and organizational skills.
  4. Current BLS certification obtained from the American Heart Association (AHA) preferred.
  5. Minimum of two years' nursing experience in a long-term care setting preferred.
  6. Prior supervisory experience preferred.
  7. Knowledge of MDS guidelines and Medicare/Medicaid regulations.
  8. Proficient in using healthcare-related software and electronic health records systems.

Ability to work collaboratively with interdisciplinary teams
