POSITION OVERVIEW:

The Health Education & Community Engagement Liaison plays a crucial role in addressing health barriers to learning (HBLs). As a Registered Nurse (RN), Nurse Practitioner (NP), or equivalent, they provide a clinical perspective for the Healthy and Ready to Learn (HRL) program. HRL is a CHF initiative that bridges the education and health system by addressing asthma, vision problems, trauma, and other HBLs known to increase school absences and decrease students’ ability to concentrate, leading to a decreased likelihood of school success. The Health Education & Community Engagement Liaison will develop health content, provide guidance to school staff, and engage families in health education for students in elementary schools and early childhood centers. This role requires a strong background in school-based health and works closely with school nurses, administrators, teachers, and families to improve student health outcomes. The focus will be on health promotion, care coordination, and health education for young students, rather than direct clinical care.

KEY RESPONSIBILITIES:

Content Development, Health Education, & Community Engagement:
  • Work closely with school health staff to ensure students’ health needs are addressed.
  • Assist with health screenings (vision, dental, asthma, etc.) and follow-up care.
  • Identify and address health barriers to learning that impact school attendance and performance. Support parents and caregivers in understanding and managing these chronic health conditions.
  • Develop health education materials, toolkits, training content and other resources for families and school staff.
  • Stay updated on best practices in school health, public health, and pediatric care to ensure content is relevant and impactful.
  • Design and lead parent workshops on key health topics, helping families understand how to support their children’s health needs.
  • Develop culturally responsive strategies to engage diverse families and caregivers in health-related discussions.
  • Collaborate with school and community partners to connect families to local health resources.
Advocacy & Evaluation:
  • Serve as a liaison between schools, families, and healthcare providers to ensure a coordinated approach to student health.
  • Advocate for school policies that promote student health and well-being.
  • Provide guidance to schools on best practices for managing student health concerns within the educational setting.
  • Document student health trends, referral outcomes, and program impact to inform future health initiatives.
  • Contribute to grant reporting and program evaluation by tracking key health indicators and student success measures.
  • Ensure compliance with health regulations, confidentiality policies (HIPAA, FERPA), and best practices in school health.

  • This part-time opportunity offers a flexible schedule of up to 20 hours per week during school hours, specifically Monday through Friday between 8:00 AM and 3:00 PM.
  • The work environment is hybrid, involving a combination of remote work and on-site presence.
  • Please note that travel between partner schools within New York City will be a requirement of this role.
  • There may be an occasional evening or weekend workshop that you would need to attend.
Location: Children’s Health Fund provides a hybrid onsite/remote work environment. This role must be based in the New York City metropolitan area in order to attend scheduled training at NYC locations or required meetings at CHF headquarters.
