Family Medicine - Physician at Our Organization - Chicago

This position entails delivering comprehensive healthcare to individuals with various complex and chronic medical issues. The focus is on managing transitions between different care environments and reducing the likelihood of hospital readmissions. The physician collaborates with a diverse team, applying evidence-based practices within a patient-centered medical home framework. Key responsibilities include addressing acute exacerbations, coordinating care strategies, and educating patients on managing chronic illnesses and advanced directives.

Key Responsibilities:

Complex Clinical Care:

  • Focus on enhancing the health of patients with multiple complex and chronic conditions.
  • Lead and collaborate with a multidisciplinary team (including social work, case management, behavioral health, pharmacy, and nutrition), process feedback, and be receptive to care plans from team members.
  • Create robust transitional care plans and monitor patients throughout their care continuum, including discharges from hospitals, emergency rooms, skilled nursing facilities, and home care.
  • Engage with various care facilities (hospitals, ERs, skilled nursing facilities) to ensure appropriate discharges to lower levels of care or back to our organization. Familiarity with inpatient medicine and discharge processes is essential.
  • Address acute exacerbations of chronic conditions within our facility, such as administering Lasix for CHF, IV fluids for dehydration or renal failure, IV antibiotics for infections, and nebulizer treatments for COPD or asthma. Must be able to determine when treatment can occur at our facility versus when a higher level of care is necessary.
  • Accept same-day or next-day urgent appointments for patients recently discharged from hospitals, ERs, or skilled nursing facilities who require intensified physician or case management services to ensure resolution of acute episodes and prevent readmissions.
  • Maintain vigilance in tracking patients and their conditions to avert ER visits, hospital admissions, and readmissions.
  • Understand the patient-centered medical home (PCMH) model, utilizing specialist care only when necessary and not as a substitute for a skilled primary care physician.
  • Educate patients about chronic disease progression and the importance of advanced directives when applicable.
  • Practice evidence-based medicine in alignment with the PCMH model.

Candidate Qualifications:

  • Licensed physician in the relevant state.
  • Board certification in Internal Medicine or Family Practice.
  • BLS certification is mandatory.
  • DEA registration.
  • Passionate about serving underserved populations.
  • Ability to work collaboratively without ego, fostering successful relationships with team members and partners.
  • Experience working within a multidisciplinary team with ongoing collaboration across provider teams.
  • Comfortable using electronic medical records (EMR).
  • Strong problem-solving skills and creativity in developing solutions.

Preferred Qualifications:

  • Experience in population health management.
  • Background in working with patients who have complex psycho-social and medical needs.

Working Conditions:

This role is performed in a professional office environment, utilizing standard office equipment. The position may require wearing specific uniforms, such as lab coats or scrubs, and using protective gear like gloves and masks. Daily exposure to patient illnesses and infections is expected, and there may be shared workspace with other staff members.

Physical Requirements:

  • Ability to communicate effectively and share accurate information consistently.
  • Capacity to remain stationary for extended periods.
  • Frequent operation of computers, keyboards, copiers, fax machines, phones, and other office equipment.
  • Job may involve physical demands at times.
  • Repetitive movements may be required.
  • Occasional lifting of objects weighing up to 20 lbs.
