Day to Day :

Schedule: M-F, 9 hours (includes lunch) between 7:30am-5:30pm

Seasonal Responsibilities

Summer:

  • Conducting family intakes
  • Accepting students into the program (must attend 2 recruitment events per month, can be outside of working hours and are paid)
  • Collect and organize family PPW
  • Assessing family needs and establishing goals
  • Medication review
  • Organize case load for the school year
  • Rapid pace for first two weeks of the school year

School Year:

  • Meeting with Families
  • Follow up with families if student attendance is low
  • Conducting 45 day deadlines per case (following up on goals, updating student medical records, referrals to community resources, conducting assessments, updating nutritional information, strengths and needs tests, documentation, and reporting)

Must Haves:

  • Bachelors degree in social work, psychology, sociology, criminal justice or other human services related field.
  • 2+ year of progressive case management experience.
  • Experience supporting both youth and families
  • Knowledge of local, state, and federal community resources in the Indianapolis area.
  • Valid drivers license and personal vehicle

 

A client is seeking a Family Advocate to support youth and families with Case Management services in Indianapolis, Indiana. The Family Advocate will manage a caseload of 40-60 by conducting intakes of new youth/families, assisting with program enrollment, completing assessments, making referrals and connections to community resources, goal setting and follow up, and participating in program recruitment.
