Position Summary:

As a Care Partner for Dialysis Patients , you will play a key role in supporting individuals with chronic kidney disease throughout their dialysis journey. This role includes providing pre- and post-treatment support, assisting with daily activities, and ensuring patient comfort and safety. You may be matched with private families in home/ senior living or dialysis facilities through the Herewith platform.

Key Responsibilities:

  • Provide personal care and assistance with daily living activities including hygiene, mobility, and meal prep
  • Monitor patients for signs of distress post-dialysis (fatigue, dizziness, blood pressure fluctuations)
  • Offer emotional reassurance and companionship to improve patient morale and reduce anxiety
  • Coordinate with family members, nurses, or facility staff regarding patient observations or needs
  • Assist with light housekeeping and environmental safety to support recovery and reduce health risks
  • Follow established protocols related to renal care support and infection control

Qualifications:

  • Previous experience working with dialysis patients, in senior care, or in a medical assistant/CNA/PCT role
  • Understanding of dialysis treatment routines, renal diets, and post-treatment care best practices
  • Strong observational, communication, and documentation skills
  • CPR/First Aid certification preferred
  • Certification as a CNA, PCT, or similar a plus, but not required for non-clinical assignments
